BIRTH CERTIFICATE (rodný list)

What

The birth certificate is a public document issued by the Registry office (Matrika) at the birth of the child. It includes data such as the date of birth, place of birth, name and surname of the child, details of father and mother of the child. The birth certificate is issued by the registry office, in the territory of which the child was born.

Where

The birth certificate of the child can be issued by the Registry Office of the city or municipal office at the place of birth of the child. In most cases, it is the relevant registry, which includes the hospital where the child was born.
